These three main factors are hue, value and chroma.1 These are the three main characteristics of color. The dictionary definition of hue is this: The property of colors by which they can be perceived as ranging from red through yellow, green, and blue, as determined by the dominant wavelength of the light. So what does that mean? Essentially it means that hue is the main color or combination of colors present. If a shirt is red, the hue is red. If a car is green, the hue is a combination of yellow and blue. It is the quality that distinguishes one color from another.1 With teeth, the differences in hue are pretty subtle. After 4 days of treatment, she complained of a foreign body sensation in the eye. The eye was red and photophobic, and she stated that the Voltaren burned more upon instillation. She was told to refrigerate the Voltaren to reduce the burning sensation and to continue treatment. She presented 29 days after surgery with 50% tissue loss. CASE 4 A 71-year-old diabetic man with systemic hypertension was treated with Falcon and 1% prednisolone given 6 times daily following cataract surgery. He experienced discomfort and hyperemia on postoperative day 7, and he noted decreased vision on postoperative day 9. Perforation occurred on postoperative day 11 and combivent.INTRODUCTION Attention deficit hyperactivity disorder ADHD ; is among the most commonly occurring behavioural disorders in children and adolescents [2, 3]. The principal features are inattention, hyperactivity and impulsive behaviour [4]. The International Classification of Diseases ICD ; and the Diagnostic and Statistical Manual of Mental Disorders DSM ; include definitions for ADHD. DSM-IV define ADHD according to the presence of either signs of inattention or signs of hyperactivity impulsivity. Three subtypes are defined: predominantly inattentive type, predominantly hyperactive impulsive type, and combined-type [2, 3]. Diagnosis of ADHD by DSM-IV requires that the following diagnostic criteria are met [2, 3, 5]: 1. Some not all ; of the signs of impairment are pervasive i.e. present in more than one setting, e.g. at home, school, socially ; for at least six months. 2. Signs of impairment are present before the age of seven. 3. Impairment has been evident in social and academic functioning. 4. The degree of symptoms is developmentally inappropriate. 5. The signs are not better accounted for by other mental disorders. The ICD-10 diagnosis of Hyperkinetic Disorder HKD ; is sometimes used in the UK. HKD is broadly similar to severe combined-type ADHD and diagnosis requires the presence of all three core signs inattention, hyperactivity and impulsiveness ; before the age of six. These symptoms should cause impairment and be present in two or more settings [2, 5]. Estimates of prevalence vary depending on the population sampled, method and diagnostic criteria used. In three studies in English populations, prevalence rates of 25% were seen [2]. The underlying causes of ADHD are unknown although there may be a genetic component [4]. Prevalence is higher for boys than girls [6] with ratios varying from 3: 1 to [4]. The National Institute for Clinical Excellence NICE ; estimated the prevalence of ADHD to be around 5% of school-aged children, aged six to 16, 345, 000 in England and 21, 000 in Wales ; with about 1% having severe disease HKD ; . Not all children who meet the diagnostic criteria for ADHD are diagnosed [5]. ADHD remains an under diagnosed condition in adults and is reported to persist in 10%60% of childhood-onset cases [3, 7, 8]. Studies suggest that ADHD is prevalent in between 1% and 6% of adults, with an average rate of 2% [3, 7, 8]. Atomoxetine is being studied in adults but a licence submission for this group is unlikely in the near future. Follow-up studies indicate that children with the disorder are at risk of developing other disorders in childhood, adolescence and adulthood including antisocial personality disorder, substance abuse, alcoholism and criminality [2, 8]. 3 There are a number of current guidelines available for the assessment and management of individuals with ADHD. The Scottish Intercollegiate Guidelines Network SIGN ; has published guidance [2] and NICE has issued specific guidance on the use of methylphenidate [5]. NICE recommends that diagnosis is based on a timely, comprehensive assessment conducted by a child adolescent psychiatrist or a paediatrician with expertise in ADHD. It should also involve the child, parents, carers, and the child's school, and take into account cultural factors in the child's environment [5]. Aims of treatment of ADHD are to reduce inattention, hyperactivity and impulsivity, and to improve psychosocial and educational functioning of those affected, with minimal side effects [4]. This is currently achieved using a range of social, psychological and behavioural interventions [5]. Drug treatment is generally recommended as an adjunct to nondrug interventions. The mainstay of drug treatment to date has been use of psychostimulants -- methylphenidate, dexamfetamine and, prior to its withdrawal, pemoline. These generally produce an improvement in core symptoms in about 70% of cases [9]. The problems of inadequate response in a substantial minority, intolerance to side effects e.g. insomnia, tics, etc ; , and concerns about abuse and medico-legal issues of treatment with controlled substances in a community setting have led to the search for alternative agents [8, 9]. Tricyclic antidepressants have been used in the management of the hyperactive and impulsive symptoms in treatment-resistant patients, but they are not licensed for the treatment of ADHD [9].
